Understand the report
What is Florida’s DWC-25?
The Florida Workers’ Compensation Uniform Medical Treatment/Status Reporting Form, numbered DFS-F5-DWC-25, communicates treatment and medical status to the workers’ compensation carrier. Its scope includes treatment-plan authorization requests, maximum medical improvement (MMI), and permanent impairment ratings. Read Florida DFS’s overview.
- Treatment & status
- Clinical determinations, proposed care, and changes in the treatment plan.
- Work capacity
- Functional limitations, restrictions, and return-to-work status.
- MMI & impairment
- MMI dates and permanent impairment information when applicable.
The form and the clinical record
The DWC-25 does not replace your visit note.
Florida’s official completion instructions are explicit:
“The Form DFS-F5-DWC-25 does not replace physician notes, medical records or required medical billing reports.”
The instructions also call for clinical records to supply additional service details and remain consistent with the form. A SOAP note is one practical structure; these instructions do not prescribe SOAP headings or a separate physical sheet.

The standardized treatment and status report for the claim.
The visit note
The encounter’s clinical story, including symptoms, examination, reasoning, care provided, and follow-up.
Our workflow recommendation: keep both associated with the same encounter. Completing the state form should not be treated as finishing the clinical documentation.
Available in InjuryMGMT
Start with the form.
Build the SOAP note with AI.
InjuryMGMT uses AI to help generate a SOAP note draft from the clinical information captured on your DWC-25, together with additional visit details you supply. You have a starting point to review and complete, rather than a second document to write from scratch.
Complete the preloaded DWC-25.
Work through the form questions and review the original PDF. The form is already set up and tested in InjuryMGMT.
Add the encounter details.
Supply relevant history, examination findings, treatment performed, and clinical reasoning that the form does not capture.
Review your AI-assisted SOAP draft.
Check the subjective, objective, assessment, and plan sections. Correct inaccuracies, complete missing information, and confirm the note matches the DWC-25 before signing.
A draft supports your documentation.
A completed form cannot supply findings that were never documented. The treating clinician remains responsible for the accuracy, completeness, and authentication of the final visit note. AI-generated text requires clinical review.
Before you get started
A few practical questions.
Do I need to upload or configure the DWC-25?
No. The Florida DFS-F5-DWC-25 is already preloaded and tested in InjuryMGMT. Your team can start with the existing form workflow instead of setting up a blank PDF.
Is the DWC-25 itself a complete SOAP note?
No. Florida’s completion instructions distinguish the state report from physician notes and other medical records. Maintain clinical documentation that describes the care provided and agrees with the DWC-25.
Can InjuryMGMT help create a SOAP note from the DWC-25?
Yes. InjuryMGMT uses AI to help draft a SOAP note from the clinical information entered on the DWC-25 and any additional visit details you supply. The treating clinician reviews, corrects, and completes the draft before using it in the medical record.
Does every Florida workers’ comp service require a DWC-25?
The requirement broadly covers treating physicians, with specified service exemptions and reporting exceptions. Check pages 3–5 of Florida’s official instructions for the provider and encounter involved.
