Keep the documents straight

A progress report has its own job.

California’s reporting rule, section 9785, addresses progress reports for continuing treatment and specified changes. When treatment continues, the reporting interval is no more than 45 days; other events can require earlier reporting. Check the rule for the circumstances involved.

First report & progress

Form 5021 covers the initial examination report. PR-2 covers progress reporting; a narrative alternative must meet the rule’s requirements.

Treatment authorization

Use the applicable RFA process for treatment requests. A completed PR-2 does not itself authorize care.

The PR-2 directs physicians to use the appropriate permanent-and-stationary reporting path when applicable, rather than this progress form. Find California’s related medical forms.

Before you get started

A few practical questions.

What is the California PR-2 form?

The PR-2 is California’s Primary Treating Physician’s Progress Report. It records progress during treatment, including complaints, findings, diagnoses, the treatment plan, and work status. The official DWC form is linked below.

Can I fill out PR-2 in InjuryMGMT?

Yes. The demonstration shows the California PR-2 available in InjuryMGMT, with guided questions and the original PDF side by side. Enter responses, check where they appear on the form, and review the document as you work.

Is PR-2 the same as Form 5021 or an RFA?

They serve different purposes. Form 5021 is the Doctor’s First Report; PR-2 is a progress report. Treatment authorization uses the applicable RFA process. Completing a PR-2 does not itself establish approval of treatment.

Does the demo show a signed or submitted report?

The video demonstrates entering responses and reviewing the PDF. It does not demonstrate signing, delivery to a claims administrator, or agency submission. Review and complete the report before using it in your practice.
